All patient encounters (in-person and telemedicine) are going to require portal access. 

Pre-visit paperwork will be required to be completed online via the portal prior to your child's appointment time.

If not completed, caregivers will be asked to reschedule the visit.

  • Compose a list of your health questions

  • Bring a list of current medications, including prescriptions, over the counter medication, and herbal supplements

  • Provide us with a comprehensive health history including allergies, hospitalizations, procedures, and pertinent test results

  • Compose a list of other healthcare providers you have visited, including specialists and their contact information.
